For citizens of the RA: identity card or passport of the RA; for children from 0 to 18 years old: social security card.

As a primary healthcare center (polyclinic), Wigmore Family Health Centre prioritizes both early disease prevention and treatment.
In addition to extra services provided by international health organizations, some of which are also free, the polyclinic offers free services carried out within the parameters of the state order.

For non-citizens:
  • 0–18 years: If one of the parents is a RA citizen, a scan of the parent's passport is needed, as is a scan of the child's birth certificate and passport (if available) in their original and translated form. All languages must be translated.
  • Children aged 0 to 18: if the parents are not Republic of Armenia citizens, scans of their social security card, passport, and residency permit are needed, together with the original and a scan of the child's birth certificate and passport (if available) translation. All languages must be translated.
  • Scan copies of the social security card, passport, and residency permit for those who are at least eighteen.
